I'm sure you know the HITT training principle, yes? But now, stroke rehabilitation professionals now have firm evidence to support implementing short, high-intensity interval training protocols in clinical practice (and by extension, into the community).
A study published just last week in Stroke, the peer-reviewed scientific journal of the American Heart Association noted the first randomised trial to examine a time-efficient, high intensity interval training programme to incorporate a phased and progressive approach.
An adaptive recumbent stepper was used, which was justified in that in meant more people could participate in high-intensity interval training, even those who cannot walk fast enough or long enough on a treadmill.
For us stroke survivors, it suggests that with the right support and guidance, stroke survivors can safely and effectively engage in high-intensity interval training, significantly improving their overall health and recovery.

It seems like Taiwan is the place to have a stroke. It has just introduced a pilot inpatient rehabilitation program for stroke patients, providing up to 180 days of intensive daily care to aid their recovery. 180 days!!
Under the program, stroke patients are to receive two to five hours of integrated rehabilitation therapy and training each day to improve their ability to live independently.

Petr Piokomy is a qualified specialist personal trainer in stroke and neurological rehabilitation, and has been working in this field for the past 16 years. For the last 7 of those, he's been an associate of the ARNI Institute.

On Friday, September 25, 2026, ARNI instructor Petr will be taking on 13 Valleys challenge in Lake District in effort to raise awareness and funds for ARNI. Struggling in this heat? If you've had a stroke, you may have noticed something odd; your more-affected side can actually feel a bit easier to move… while the rest of you feels wiped out. 🧠🌡️
We're into what looks like the UK's hottest summer on record (37-38°C this week, amber alerts across most of England), so it what does the heat do to you? Well, the good part first. As many of you will have felt, warmth does tend to ease spasticity. Cold of course does the total opposite; it ramps up activity in the muscle spindles (the sensors that set off the stretch reflex) and muscles in positions of spasticity tend to pull tighter, while heat settles that down. So a tight arm or leg often moves more freely in weather like this. If your affected side feels more workable right now, you're not imagining it!
But, annoyingly, after a stroke your body often doesn't regulate its own temperature as well, so you overheat faster and get fatigue very easily. And some common meds (BP tabs, diuretics, some antidepressants etc) change how you cope with heat and fluid; and dehydration thickens the blood, which is itself a stroke risk...
So you get looser muscles, but no stamina, shaky balance and poor concentration to go with them… which makes rehab training in the heat not at all easy ;).
The hot countries sorted this out long ago; across Spain, Italy, Greece and the Middle East, they do more at the cool ends of the day, and the hottest hours are for shade and rest. Not a bad model for us this week. .

These are the people who will show you how to encourage real plastic change, yet understand that it is vital that you also develop some real-life coping strategies… fast. So which is best: weight training or bodyweight training?
The truth is that bodyweight exercises and weight training can actually be combined into the same workout very effectively; it does not have to be an ‘either/or’ situation. BOTH training styles are useful, BOTH can make you very strong, and (in Tom's opinion) BOTH can be a part of your training programme as you become more advanced. A combination of weights and bodyweight training leads to the best overall development and they can complement each other extremely well.

Can you recover from stroke if you can’t find anyone to help you? ABSOLUTELY! You can still self-rehab very successfully indeed, but to start, you’ll appreciate that knowledge is power. Which is why therapists and specialists instructors are well worth ‘using’, as they can provide you with the grounding that you probably need to understand how to reach ‘stages’, get to stages, make your own stages etc. And you need to do your knowledge accumulation (if you can.. ).

Deep down, you have to believe that change is possible. Hope is a powerful drug that makes therapy so much more effective. Some people hear from their doctors that they won’t get any better after 6 weeks, 3 months, 6 months, 1 year… These timelines are untrue and can destroy hope. Hope alone won’t make you better, but without hope, progress is rare.
Always remember there are online videos and books on our website that will help give you hope in your recovery by providing you with exercises to help you.

When they don't follow you up how can they ever know.....you're discharged and that's it. This is why stroke care is so outdated in the UK and probably elsewhere - a lot of other conditions get 6 month or annual follow ups, but not stroke (in the main - there are some exceptions). But stop giving out information that leaves you thinking you have a limited window to regain
